The PathFinders' Aftercare Program is designed to provide an ongoing counseling program for patients who have completed treatment in Residential or Intensive Outpatient Programs. The Aftercare group provides support to those who have remained chemically abstinent for extended periods of time. The PathFinders' recognizes that most often relapse occurs during the initial six months following treatment. Participants in the Aftercare group review dynamics leading to relapse and explore how to effectively cope with abstinence based issues.

The PathFinders' also recognizes that employers and other interested parties have a need to monitor the success of persons involved in the Aftercare Group. The PathFinders provides follow up correspondence to employers and other agencies to aid them in this monitoring process. Attendance in The PathFinders' Aftercare Program is suggested for at least six months following treatment. However there is no limit to how long a person may attend the Aftercare Program. All persons who have successfully completed treatment at The PathFinders are encouraged to become a member of the Alumni Association.
